To get started, please fill out the form below. Remember to write down your username and password for future use. You will need this information to access My Healthcare Advisor at http://myhealthcareadvisor.com.


Enter full name, eg. John Smith.

If your employer is sponsoring your My Healthcare Advisor membership, they should have supplied you with a referral code. Entering this code will bypass the credit card payment process.

Enter a username (e.g. "jsmith"). No spaces or special characters. Usernames and passwords are case sensitive; make sure the caps lock key is not enabled. This is the name you will use to log in to the site.

Enter an e-mail address. We respect your privacy, and will not give the address away to any third party vendors.

Your zip code will be used to customize your content.
